Staff take tests from a link, with no account to create and no password to forget. Results count each person's first attempt, and you decide who gets a retake.
Equipment
Each item keeps its own history of faults, repairs and services. Service intervals set the next due date, and only the person responsible can sign a service off.

Frozen at submission
The site name, the question wording and who ran it are copied into the record. A later edit can never rewrite what was reported.
Evidence without the shortcut
Photos are asked for, never forced. Forced evidence makes "mark it as passing" the quickest way through, so reports show coverage instead.
